The vision of Health and Wellness Services is to help with the gap of health inequities throughout our community and to provide a wide range of culturally conscious care and services.
HWS has been tasked to help our community at large through our collaborative integrated program. Our community has been challenged with so many obstacles in receiving healthcare.
HWS provides exceptional primary care to our patients, along with a range of other services, from behavioral health, chronic care management, substance abuse counseling, psychotherapy, HIV counseling, psychosocial assessment for children, adolescents, adults, and families … this can be in the home , via phone or a form of telehealth communication.
“Payment for services:”
HWS services are fully credentialed with most insurance plans. We accept any forms of payment made by the approved insurance company.
All services can be performed at home, in the office, or virtually.
